Health history form
For all patients including those whose first language is not English we have provided the following health information forms so that you may print and fill them out in the comfort of your own home before you arrive for your appointment. These forms must be filled out by all new patients as well as patients who have experienced significant health changes in the last 6 months.

Insurance
Due to the new privacy act (PIPEDA) and changes in office regulations, all new patients must pay our office on the day services are rendered. For your convenience we accept VISA, INTERACT, CHEQUE and CASH.
An insurance form will be submitted on your behalf and you will be reimbursed directly by your insurance company.
